10/21/2022 – Mental Health Community Partners: MHANYS Innovative Programming Geared to Supporting Recovery of Adults and Youth Aged 12 and up with Serious Mental Health Issues: No Cost Training


The Mental Health Association in New York State, Inc. (MHANYS) has received funding from the Office of Mental Health to launch the Mental Health Community Partners Program (MHCP) across New York State.

The MHCP Program, founded as an extension of MHANYS’ signature CarePath™ Program, supports the recovery of adults and youth aged 12+ with serious mental illness/serious emotional disturbance as they transition from any type of treatment setting to the community.  The 90-day transitional program engages individuals in recovery and their families or other chosen supporters throughout pre and post discharge stages.

With the support of a certified Mental Health Community Partner, individuals and their families/supporters are guided in creating a comprehensive care planning strategy to successfully transition to the community and prevent re-hospitalization.

MHANYS is offering Mental Health Community Partner Certification and Recertification Trainings at no cost.   Leadership from participating agencies must sign an MOU committing to being a Partner site. Leadership selects staff for certification in the MHCP model. Partner sites with certified Community Partners trained during this project receive future ongoing certification at no cost.
