Here at MHANYS we are excited to share that the New York State Office of Mental Health (OMH) has featured MHANYS transition support tool, CarePath™ in their latest newsletter. MHANYS CarePath™ has been moving across the state certifying CarePath™ Coaches in this new model that supports individuals and their family members or other identified supporters in defining what constitutes whole-person and family health and well-being.
